With both the prevalence and cost of long-term conditions (LTCs) continuing to grow, it is clear that doing more of the same will not be good enough if services are to be sustainable into the future. The figures involved are pretty staggering: in total, around 70% of the total health and social care spend in England goes to caring for people with LTCs. There is a clear priority to transform the way care services are delivered to people living with a LTC, not only to give them the best quality of life but also to contribute to the current financial imperative of saving up to £20bn.

LTCs are the biggest challenge facing the NHS, now and for the future; therefore, it is imperative that plans are put in place to help address the health and social care issues for people living with a LTC and the additional costs for the NHS. A new approach is needed, one that puts the patient at the heart of decision-making, that supports self-care and that embraces innovations, technologies and ways of working.
