As a former GP, Dr Kate Fallon is passionate about improving services for patients and families and has championed the concept of integrated care, both locally and nationally. She practised as a GP for over 20 years, prior to accepting a full-time post as Medical Director for her local PCT in 2004.
Kate was an early adopter of clinical audit and significant event analysis in her own practice. This led to pioneering the use of guidelines and clinical care pathways, which now routinely underpin the successful delivery of integrated care, crossing professional and agency boundaries.
As CEO of one of the new community trusts in the NHS, Kate is working with colleagues to establish an evidence-based set of quality indicators for care delivered out of hospital.

Chair, Cumbria Clinical Commissioning Group

Hugh Reeve is a GP partner in Grange-over-Sands and is also Chair of the Cumbria Clinical Commissioning Group. This is one of the largest CCGs in the country and with its partners is embarking on a challenging development programme for primary, community and social care. The CCG recognises that services in the community require investment and significant change before any real shift in services from a hospital setting can occur. Previously, he has done lots of interesting things in medical management, education and academia, mainly in the Manchester area. Married (very happily!), with four daughters, along with dogs, cats and horses, and not much spare cash!

For the acute trusts to provide services for those most in need and to deliver more patient preferred treatment closer to home, there needs to be a rethink of the current systems. Creating a strong integrated platform for primary, community and social care is necessary if there are to be cost-effective improvements in patient experience and outcomes.

General Practitioner in NHS Manchester, GPwSI in Cardiology, Clinical Director for Central Manchester Clinical Commissioning Group

Dr Ivan Benett has been a GP in Central Manchester for 25 years. He has held various educational posts and has been PCG Chair and then Chair of the PCT Professional Executive Committee until 2005. Following this, he gained a postgraduate diploma in cardiology and became a GP with a special interest in cardiology. Ivan has worked closely with the cardiac network and developed a primary care strategy and pathways for cardiology. He has been on the NICE guideline groups for Chronic Kidney Disease and Post-MI care, as well as the NICE topic expert group for Quality Standards in Heart Failure and CKD care.
Ivan is a Fellow of the Royal Colleges of GPs and Physicians. He has recently been appointed Clinical Director of the Central Manchester Clinical Commissioning Group.

Domini Gunn-Peim is developing and delivering a collaborative and partnership approach to the delivery of integrated services across housing, health and social care. CIH is working to ensure a greater recognition of the contribution of good quality housing and support to achieving improved health and wellbeing for individuals and communities, and in tackling health inequalities.
Prior to joining CIH in 2011, Domini worked at the Audit Commission as the national lead for vulnerable communities and a lead housing inspector. Domini chaired the Hospital2Home Advisory Group, is a member of the HCA Vulnerable & Older People's Advisory Group (VOPAG), and is a trustee of Care & Repair (England) and the Extra Care Charitable Trust. She has had papers published in a number of journals and participated in international projects in Ireland, Poland, France, Spain, Germany and Nicaragua.
